This is a theory on what triggers the Hinamizawa syndrome
Hinamizawa Syndrome + Takano + Beliefs or Hinamizawa Syndrome + Beliefs = hallucinogenic reality
Each character has 1 thing in common, their beliefs is what drove them into insanity.
But in Tatarigoroshi-hen, I did'ent understand this well. If Keiichi believed he killed Teppi (Satoko's Uncle). Then why did his friends say he was at the Watanagashi Festival? Is what they said a hallucination? Or is killing Teppi an hallucination? If so then why did'ent Keiichi remember being at the festival? Any Thoughts?

Alright, I'm here to clear up a few things since I think I've figured out most of the questions you have...
The Hinamizawa Syndrome that had infected Tomitake was agitated by a solution/drug that Takano created. She talks about it numerous times in Higurashi Kai and in one of the episodes where Tomitake is cornered inside the hotel, he is injected with a syringe filled with a drug.
Rena's Hinamizawa Syndrome was agitated only because she left Hinamizawa. On the other hand, Satoko went through a lot of stress after living with her aunt and uncle and the disappearance of Satoshi. It seems that, normally, people do not recover when the syndrome is agitated by stress.
Keiichi became uneasy with the knowledge that a brutal murder occured in Hinamizawa in the past. He furthermore became suspicious of his friends, which also helped agitate the Hinamizawa Syndrome. This was severe stress, though, and he only came down with level 1 or 2, and then even further.
From what I gather, there was no needle in the ohagi that Rena made. Considering the size of the ohagi and the fact that it is handmade, it is almost impossible for someone not to notice a needle in it while making it. Also, from the Higurashi game (the original), Keiichi first believes that it is a hair. As time passes, however, he beging to overexaggerate the qualities of whatever he found in the ohagi. He begins to think it was long, thin, and sharp (all of which someone could consider characteristics of hair, except sharp maybe..). Keiichi later asked his Mom if she had found a needle while cleaning up the ohagi he threw on the walls. She hadn't. Maybe she had simply not noticed, or maybe it was really a hair.
I'm guessing that many of you haven't read/played the original Higurashi game. There was so much to put into the first season of Higurashi that the producers had to cut out many important parts/details in order to make it fit into one season.
You can buy the game from a whole bunch of places..I don't remember any off the top of my head...You need a translation patch for it though, and it currently isn't completed. As of now, only up to Onikakushi day 14 (out of 15) have been translated.

AlbireoTheMad, reffering to your first post about how Takano may have found a way to agitate the disease, i dont think it's that, but rather that she found a way to isolate the disease, which would make it's effects quicker and more prominent. Then about the first arc of koro ni, the needle in the Ohagi is actually just a sthingy. Then when Rena and Mion are "trying to inject Keichi with the syringe, it's actually just a marker that they were going to draw on him with. Basically Keichi had Hinamizawa syndrome (level 4) the whole time and imagined everything.

Hmm , Honestly I figured that the entire Mion Rena thing with the Needle was one massive delusion.. Honestly you dont go from being held back and about to be injected to going over picking up a bat and beating two people who are strong enough on their own to death. They likely never saw it coming. He was Level 4 for a decent period before he killed though. It was rather a shame..
I just find the entire Syndrome and the subject matter very interesting. I think it takes a specific type of stress to.. Not just "stress" but a distinct type like mixing doubts abuse the extremes.

I think I have an answer for what happened in the third arc of the first season. Since Mion had a feeling of what Keiichi planned to do so she conviced the group to lie so he could be protected. Alternatively he could have gone into stage four hinamizawa syndrome and hallusinated what everyone was saying which explains his murderous behavior and eyes while killing satoko's uncle. Then after dropping keiichi off at his house takono put her plan into action digging up the body and making it seem like nothing was ever there. As for the hot topic discussion about the needle, since keiichi was already in the late stages of level 3 at this point anything as much as a level of spicy taste higher then what he expected could set him off. Rena probably made hers more spicy to make it a bit noticeable but keiichi thought the hot flavoring was like jabbing on his tongue so he thought it was a needle. But what i wanna know is even if rena heard keiichi's phone conversation in arc 1 why would she leave instead of talking about it with him and how did she get out of the house. I mean his dad made the tea and keiichi wasn't yet in level 3 so hillusinations weren't possible at that point.
